Anxiety Assessment Worksheet

The Lutheran Church of Canada has been kind enough to post an 8-page anxiety assessment worksheet, partially lifted from David Burns's Feeling Good Handbook. Check in with yourself about:
  • Fatigue
  • Anxious Thoughts, Feelings, and Physical Symptoms
  • Physical Activity and Health
  • Life Changes (a rating scale--e.g., Divorce = 73/100)
