anxiety assessment worksheet, partially lifted from David Burns's
Feeling Good Handbook. Check in with yourself about:
- Fatigue
- Anxious Thoughts, Feelings, and Physical Symptoms
- Physical Activity and Health
- Life Changes (a rating scale--e.g., Divorce = 73/100)
